    Title: 病情複雜性與時間充裕性對門診藥師角色認知影響之研究

    Abstract: 由於整體環境的快速改變與全民健保制度的實施,使民眾有更多元且富彈性之就醫選擇,台灣的醫療產業市場競爭愈趨激烈,因此,醫療產業逐漸引進企業經營管理的理念,開始著重整體形象與服務品質,以提升顧客滿意度將病患。 本研究將以中區醫院為例,將針對病情複雜性與時間充裕性來探討病患是否會因為此兩變數而對於門診藥師的期望有所影響,運用情境分析法(Scenario Analysis),以病情複雜性與時間充裕性此兩構面來建構病患與藥師可能發生的情境,全面實施問卷調查,評估病患與藥師之間的認知差異。研究發現如下:(1)從病情複雜性層面而言,病患會因病因複雜程度的影響,而對專業門診藥師之信心而產生影響。在時間充裕性低時,病情複雜度高的病患對於專業門診藥師之信心、合作與期望是高於病情複雜性低之病患;在時間充裕性高時,病情複雜性低之病患對於專業門診藥師之信心、合作與期望是高於病情複雜性高之病患。(2)從時間充裕性層面而言,病患對於專業門診藥師之信心會因時間充裕性而產生影響,且是正向的影響,即是當時間充裕性愈高,則病患對於專業門診藥師之信心愈高;反之,則亦然。在病情複雜性高時,時間充裕性高的病患對於專業門診藥師之信心、合作與期望會高於時間充裕性低之病患;在病情複雜性低時,時間充裕性高的病患對於專業門診藥師之信心、合作與期望是高於時間充裕性低之病患。(3)病患對於專業門診藥師之期望,與病患之信心呈現正向的影響。即是當病患之信心愈高時,對於專業門診藥師之期望便是愈高;反之,則亦然。病患與專業門診藥師之合作意願,與病患之信心呈現正向的影響。即是當病患對於專業門診藥師之信心愈高時,與之合作的意願便愈高;反之,則亦然。故,當病患對於專業門診藥師之信心與其期望與合作呈現出正向的關係。
    Competition in the medical community has become very fierce. Business management concepts have been brought into hospital administration in Taiwan. Pharmacy services are a major component of the medical treatment provided in hospitals. This research takes hospitals in central Taiwan as examples. We focus on the diseases complexity and time availability to investigate into patients about the effect on the role expectation for the outpatient pharmacists. We use the Scenario Analysis and construct the possible scenarios about the patients and pharmacists, and implement questionnaire investigations into the difference about role expectation to outpatient pharmacists between patients. We find some point as follows:(4) In diseases complexity dimension, the patients not impact confidence by complexity of diseases for .outpatient pharmacists. In low time availability, the patients in high diseases complexity have more confidence, cooperation and expectation than in low diseases complexity. In high time availability, the patients in low diseases complexity have more confidence, cooperation and expectation than in high diseases complexity. (5) In time availability dimension, the patients have positive effect from time availability for outpatient pharmacists. In other words, when time availability is higher, the patients have more confidence for outpatient pharmacists; on the contrary, when time availability is lower, the patients have less confidence for outpatient pharmacists.(3)The patients’ confidence has positive impact on role expectation for outpatient pharmacists. In other words, when the patients have more confidence for outpatient pharmacists, they have higher expectation for outpatient pharmacists. The patients’ confidence is positive impact on cooperation for outpatient pharmacists. In other words, when the patients have more confidence for outpatient pharmacists, they have strong will to cooperate with outpatient pharmacists. Consequently, the patients’ confidence is positive impact on cooperation and expectation for outpatient pharmacists.
