如何透過資源分配改善急診壅塞是醫院管理者需要重視的議題;本研究目的是探討急診設施對於護理師和急診留觀床資源配置的影響,因此本研究試圖使用活動關係圖來了解急診設施間的關係與重要性,並且使用射頻識別(RFID)技術收集護理師在設施間的走動的距離。此外,透過系統模擬在台中某醫院的急診中進行實驗,以減少護理師的走動時間,改善醫院急診內與設施間的距離。實驗結果表明,手術診療室需要與外科留觀區合併,並且與第二留觀區一起交換。可以減少約4%護理師的工作時間。此改善急診的護理師總工作時間一年可以節省852小時。模擬實驗表明,增加5張留觀床可以減少70%的病患等待留觀床的時間。因此,不同急診設施的配置會影響急診人員的工作量,急診護理師的配置須納入人員走動時間的因素;最大等待留觀時間是可以用於評估走廊護理的情形,並作為增加多少留觀床數量的參考依據。 How emergency department (ED) overcrowding is improved through resource allocation is an issue that hospital administrators need to pay attention to. The purpose of this study is to explore the impact of ED facility on the allocation of nursing staff and ED bed resources, so this study attempts using Activity-Relationship Diagram to understand the relationship and importance of units in ED. Collect the traveling distance of nurses between one unit and the other by using Radio Frequency IDentification (RFID) technology. In addition, the experiments were conducted in the ED of Hospital by the system simulation.The experiment results demonstrate that surgical consulting room needs to merge with the surgical observation section and switch with the observation section II. Then, the working time of nurses can be reduced about 4%. Also, total working time of nurses in ED can saved 852 hours for a year. The simulation experiment suggests that adding five beds can reduce 70% of average patient waiting for the ED beds.Therefore, the configuration of different emergency facilities will affect the workload of the nursing staff, the configuration of the emergency nursing staff must be included in the traveling time of the personnel; the maximum waiting time can be used to assess the providing care in ED hallways, and as the basis for increasing the number of ED beds.
