    Abstract: 面對世界快速變遷的環境,醫療生態丕變,醫療院所需注重管理效能,增進照護成效與提高顧客滿意度。病患在就醫的過程中,產生許多醫療體系的等待時間,醫療服務時間已成為醫院評鑑項目中醫療服務品質之重點指標。醫療影像檢查的需求量倍增,檢查等候時間過長,延後了疾病診斷確立的時間,更影響進行治療時機點。在醫療資源有限的情形下,若能將檢查預約排程時間縮短,將減少病患等候檢查的時間,並且能達到充分利用醫療資源、降低儀器及人員的閒置。本研究經由超音波檢查預約排程的相關資料,透過專家訪談,從醫療機構、病患、人力等各層面,從中探討影響超音波檢查排程的相關因素。檢查排程等候時間是由若干因素所影響,從預約排程規則與額度分配、病房患者的檢查流程,病患對檢查時間的需求及是否依排程時間赴約檢查,人力與技術專業,種種因素都牽繫著檢查排程的長度。根據本研究結果顯示醫療檢查需求影響排程的長短,病房檢查流程可做精實醫療改善。病患對於檢查時間要求傾向於上午時段,失約是影響檢查完檢率的重要因素也間接影響排程,其中男性失約率高出女性3.24%、長時間預約排程(>90天)失約率最高、乳房攝影加乳房超音波的檢查項目失約率最低。放射師人力與檢查量成正比,人力不足造成排程拉長及人員加班的情況。
    Under the rapid growth of the world environment, the medical ecology is changeable. The hospital needs to focus on management effectiveness. Improve the effectiveness of care and improve customer satisfaction. The patient has a lot of waiting time during the course of medical treatment. Medical service time becomes one of the important indicators of the quality for medical care hospital accreditation. The demand for medical imaging continues to increase, the waiting time for examination is too long, delayed diagnosis established time, more affect the timing of the treatment.With limited medical resources, If the scheduled appointment time is shortened, the patient can reduce the waiting time of examination, it can also achieve the full use of medical resources, reduce the waste of equipment idle and human resource. In this study, we use the data of ultrasonic scheduling, and through expert interviews, to explore the factors affecting the scheduling of ultrasonic examination from the medical institutions, patients, and human resources. Scheduled waiting time is affected by a number of factors. From booking schedule rules and quota allocation, ward patient examination process. Patient preference for examination time, and whether to go to the appointment inspection according to the scheduled time. Manpower and professional technology, all factors are related to the length of the schedule.According to the results of this study, the examination requirements affect the examination scheduling. The ward inspection process can be improved by Lean Healthcare. Patient tend to a.m. for examination, didn’t show up is an important factor affecting the examination rate and indirectly affects the examination scheduling. The male didn’t show up rate is 3.24% higher than women. The long-term appointment schedule (>90 days) has the highest rate of didn’t show up, and the mammography plus breast ultrasound items have the lowest rate. The radiological technologist's manpower is directly proportional to the amount of examination, and short-staffed causes the scheduling to be extended and the staff to work overtime.
