Abstract: | 摘要本研究目的旨在探討護理人員對護理品質的認知及其影響因素之相關性。本研究以台中市參加新制醫院評鑑結果為合格、優等、特優等級,各一家醫院之內外綜合科病房全體護理人員為對象,以九個構面的護理照顧來進行問卷調查,共得有效問卷409 份。資料利用SPSS for Windows 13.0 統計軟體進行描述性統計及推論性統計。研究結果,護理人員自覺重要度整體得分平均 4.46,在年齡層、職稱、能力進階、工作年資,與多項構面有顯著相關;自評執行能力整體平均分數4.05,以護士得分最高,資淺及未取得能力進階認證的護理人員自評執行能力得分較低。自覺重要度與自評執行能力呈正相關(r>0)。護理長對單位護理人員評分,高於護理人員之自評能力,22 項有顯著差異(p<0.05)。影響護理人員護理品質的相關因素之迴歸分析,進入迴歸方程式的顯著變項,主要有職稱、能力進階、醫院屬別、參加評鑑次數、曾參加新制醫院評鑑。特優醫院的護理人員自覺重要程度、自評護理照護執行能力高於優等或合格醫院。有參加醫院評鑑次數4 次以上及參加新制醫院評鑑並已完成評鑑的護理人員,自覺重要程度及自評護理照護執行能力高未參加者及不知道者。研究結果可提供護理主管人力資源管理及護理人員工作生涯規劃,做為提升護理品質的參考。關鍵詞:護理人員、護理品質、新制醫院評鑑 ABSTRACTThe purpose of this study was to determine the factors affecting nursing staff’sperceptions of the quality of nursing care and to analyze the correlations among those factors.A questionnaire survey, that included nine dimensions of nursing care, was administered toall nursing staff ,working in the integrated medical and surgery wards of hospitals. Thenursing staff worked in different hospitals of central Taiwan that were rated as “qualified”,“excellent” and “superior” in the Reformed Hospital Accreditation scheme. A total of 409valid questionnaires were collected and analyzed using descriptive Statistics and StatisticalInference software (SPSS for Windows, version 13.0).The results of the this survey showed that the total mean score of self-reported quality ofcare awareness in nurses was 4.46. Individual characteristics such as age, job position,nursing clinical ladder level, and number of working years influenced some dimensions of theself-reported quality of care awareness in the nurses. The total mean implementation capacityscore was 4.05.Results showed the staff who’s job positions is nurse scored hightest, whereasnurses at of lower age and the lowest level on the clinical ladder scored low. Nursing qualityof care awareness and implementation capacity were found to be positively correlated (r> 0).The scores of unit capacity head nurses’ in the ward and the overall implementation rate ofnurses were higher than those of the implementation capacity obtained throughself-assessment, with 22 items showing a significant difference (p<0.05). Stepwise regressionanalysis revealed that job position, nursing clinical ladder level, hospital type, and hospitalaccreditation significantly correlated with the nursing staff’s quality of care awareness.The scores of nursing quality of care awareness and self-assessed implementationcapacity were higher in hospitals rated “superior” compared with hospitals rated “qualified”or “excellent”. Also the scores of nursing quality of care awareness and self-assessedimplementation capacity in hospitals evaluated by the Reformed Hospital Accreditationscheme were more than four times the scores in hospitals never evaluated before.The results of the study may offer human resources management (HRM), nursingdirector and nurses with valuable insights into possible areas in which the quality of carecould be improved.Keywords: Nursing staff, nursing quality, The Reformed Hospital Accreditationscheme |
